How it works — from form to first appointment
Step 1: Check your insurance and submit the form Fill out the short form below with your insurance information and basic details. This takes approximately 5 minutes. We use this information to verify your coverage before confirming your appointment.
Step 2: We verify your insurance and confirm your appointment Our team reviews your information and confirms your insurance coverage. You will hear back from us promptly with your confirmed appointment date and time. Most new patients are seen within approximately 3 days of submitting the form.
Step 3: You receive your Zoom link Once your appointment is confirmed, you will receive a secure Zoom link for your video session. No app download is required on most devices. You join from wherever you are most comfortable — your home, your parked car, your office.
Step 4: Your 60-minute evaluation Your first appointment is a 60-minute psychiatric evaluation with your provider. Your provider will spend this time understanding your symptoms, history, and goals. In most cases a diagnostic impression and initial treatment plan are established at this first session.
Step 5: Ongoing care Follow-up appointments are 30 minutes or longer and are scheduled at a cadence appropriate to your treatment plan. You have direct access to your provider through our secure messaging portal between appointments.

Self-pay rates are $200 for the initial evaluation and $100 for follow-up sessions.

Your provider is matched based on your needs and age. Dr. Jennifer Sam, DNP, PMHNP-BC sees teenagers ages 12–17 and adult ADHD patients. Darla Dane, PMHNP-BC specializes in women's hormonal psychiatry — PMDD, perimenopause, menopause, and postmenopause — and ADHD. Both providers see adults with anxiety, depression, trauma, PTSD, OCD, and bipolar disorder. If you have a preference, you can indicate that in the form.
